Instructions

Persons who represent organizations wishing to apply for a grant are invited to send a proposal at any time during the year.

1. All applications must include the following:
2. Name, address, and phone number of the organization, and the name of a contact person.
3. Proof of tax-exempt status.
4. Specific amount being requested and what it is to be used for.
5. Description of the project and what is hoped to be accomplished (objectives.) Include a statement of why it is needed and the number of people intended to serve.
6. A statement of how the project will be implemented, monitored, and how results will be evaluated.
7. The name and qualifications of the person directing the project.
8. A detailed budget of the project, including other anticipated sources of funding.
9. On-line grant submissions preferred.
